Below are samples of full size DUO WEB 8 ½ X 11laser label, the forms are available in blank and pre-printed with your logo in your color scheme. The first form is available in both 8-1/2 X 11 and 8-1/2 X 14, the top portion are labels, the bottom is perforated paper and contains the following: Rx Label, warning labels, Signature label, Legible backside Rx label, Safety label, Mailing label, Counseling information area, Two patient receipts. You may use a combination, for example you may want to print a full patient counseling leaflet on new Rx and print to a small thermal form on Refills.
